Gearbox fault 6 speed please help its driving me mad

my discovery 4 2010 has a issue with its gearbox
when cold first thing in the morning its fine, starts up and drives perfectly but when i get to the destination and i go between D and N a gearbox error flags up. i can then select R but it jolts into gear and if i go back into drive that is the same clunks into gear. if i drive away now it is stuck in one gear 4k rpm at about 50mph and will not change gear even in manual mode S shows on dash but no number. if you stop the vehicle now and put it into park it wont show P on the dash and when you try to turn the engine off it says select park so you have to hold the button to force the engine to stop. at this point you cant start the car as it cant see Park so you sit there for 10 minutes with the ignition on waiting for P to appear on the dash, then you can start the car but it is now locked in P so you have to stop and restart the engine to be able to select D and all is good until you take it out of D again.
where dost it get the feedback for the gear position, could this be the problem
